Our Claims Manager is standing by to guide you through the process. If you are submitting a form after business hours (M-F 9:00 am to 5:00 pm), please note that the claim will be submitted during the next business day.

Submit an Incident Report

It is important to have written incident reports on file regarding injuries, property damage, or other incidents. Keep one copy on file for your records and make sure to forward a copy to the claims department at Mackoul Risk Solutions.
